Insurance Information

At Heine, Heine, & Bowles Oral and Maxillofacial Surgery we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this, you will need to provide the appropriate information needed for filing you insurance to maximize your insurance reimbursement for covered procedures. Please be prepared to provide a copy of both dental and medical insurance cards; on occasion your dental insurance will ask us to file medical insurance first, in order to get a denial, and then file the dental insurance. We will need this information to check benefits and file insurance.

We are in-network with Anthem Dental, Cigna Dental, Delta Dental-Premier plans, Health Resources Inc, United Concordia-Elite Plus.
We will file dental insurance as a courtesy to our patients, If, we are not in your network, we ask that you pay in full on the day of service, and we will file your claim for payment and assign payment to come back to the insured. We will not file any Medicaid or Medicaid affiliates.
